What is the Medicare Prescription Payment Plan?
The Medicare Prescription Payment Plan (M3P) is a federal program launched January 1, 2025, under the Inflation Reduction Act. It allows Medicare Part D beneficiaries to spread their out-of-pocket prescription drug costs across monthly installments throughout the plan year, rather than paying the full amount at the pharmacy counter. The M3P payment plan caps annual out-of-pocket costs at predictable monthly amounts — no lump sums, no interest, no fees.
How do I enroll in the Medicare Prescription Payment Plan?
Enrolling in the Medicare Prescription Payment Plan is managed through your Medicare Part D plan or Medicare Advantage plan with drug coverage. You can opt in through GetMyMeds.com or contact your plan directly during Open Enrollment (October 15 – December 7) or at other qualifying times. Once enrolled, your plan handles the monthly billing automatically — you don't need to do anything at the pharmacy.

How does the Medicare Prescription Payment Plan work?
The Medicare Prescription Payment Plan works by removing upfront costs at the pharmacy. Once enrolled, members pay $0 for covered Part D drugs. Their plan bills them monthly for the cost-sharing they accumulate. The annual out-of-pocket maximum is divided into manageable installments, so members who take high-cost medications early in the year aren't hit with a single large payment. Plans are required to offer this to all Part D beneficiaries — and required to manage the billing, communications, and reconciliation that come with it.
Will I still need to pay anything when I pick up my medications?
No. Once you're enrolled in M3P, you pay $0 at the pharmacy for covered Part D drugs. Your cost-sharing is billed to you monthly by your plan. You'll receive a statement showing what you owe for that month based on the prescriptions you filled.
Who is eligible to participate in the Medicare Prescription Payment Plan?
All Medicare Part D beneficiaries, including those in standalone Part D plans and Medicare Advantage plans with drug coverage are eligible to opt into M3P. Your plan is required by law to offer it. There are no income thresholds or application requirements beyond enrollment in a qualifying plan.
